Termination for substantial breach by the landlord
If a landlord has broken the lease agreement in a major way, a tenant can end their lease by applying to a court or by giving a written notice to the landlord at least 14 days before they want to leave.
The notice must be in writing, signed by the tenant, and explain why they want to leave and when they plan to do so.
if the landlord objects to the termination within seven days and can prove they have complied with the order or have been given more time to do so, the notice to terminate will not be effective.

A notice to terminate must:
be in writing
be signed by the person giving the notice or the person's agent,
include the reasons for termination of tenancy
identify the premises that the notice is served
state the date on which the tenancy is to terminate
